The Office of Disability Services is committed to ensuring that all information regarding a student is confidentially maintained as required and permitted by law. Information provided to the Office of Disability Services is considered confidential and is not disclosed to a third party without the written permission of the student. Any information regarding a disability is considered confidential and is only shared within the institution when there is a legitimate reason. The Office of Disability Services follows the regulations set forth by the Health Insurance Portability and Accountability Act (HIPAA)
Student's who wish to share information about their disability or accommodations, they must sign a release of information. A release of information is a legal written document that gives permission to disclose or receive information pertaining to a specific student and their disability. Students must sign a release of information that allows the Office of Disability Services to speak to any individual on their behalf relative to their disability including their doctor, therapist, and parents. If you have questions about signing a release of information or confidentiality please contact the Office of Disability Services. It is your responsibility as a student to become informed about procedures and policies and to make use of the resources and services available to you. Not all confidential protected health information submitted as documentation can be re-released by the Office of Disability Services.
On occasion, it may be necessary to disclose the nature of the disability in the letter of accommodation in order to assist you with potential safety or emergency situations, such as fire drills, building evacuations, specific medical information, etc. Also please note that a student's file may be released pursuant to a court order or subpoena.
