To all incoming freshman or transfer students (Fall 2007): There are two forms required for all student-athletes for each academic year which need to be completed prior to coming to campus. The first is a pre-participation physical exam and the second is an insurance form. The pre-participation physical exam must be performed by a medical doctor (MD, DO) no earlier than July 1, 2007 and documented on the correct form (high school physicals are not acceptable). Pre-participation Physical Forms are available online - you can find the link under “Forms for Athletes” on the left side bar of the athletics web page. Prior to your exam, you should answer all the questions on page two of the pre-participation form completely and to the best of your knowledge. During the exam, the physician needs to complete page one after reviewing the information you submitted. Please do not wait until you come to campus - there are limited physicians in the immediate area and they do know your medical history, so their evaluation will not be as comprehensive. The insurance form (also found under the “Forms for Athletes” drop down menu on the athletics web page) should be current when you arrive on campus. If you are covered by your parent’s insurance, make sure it covers physicians in the Montgomery/Charleston area, and make sure you are still eligible to remain on their policy for the full academic year. If you do not have health insurance, I strongly suggest you inquire with agents in the Montgomery/Charleston area for affordable policies which will cover general medical issues which may occur during your time at WVU Tech. WVU Tech’s Athletic insurance is only a secondary policy which covers injuries that occur during athletic practice and games, not illnesses.
